Two rules: exactly one person (or a small leadership group) at the top, and every other person with exactly one primary manager. No one reports, even indirectly, to someone below them, because that would form a loop.
Secondary relationships such as a matrix project lead do not break the tree. They are drawn as dotted lines on top of it, while the solid lines still form the tree.
